This form has been created to help nominate patrollers for our annual patrol awards given at the Spring picnic.
A copy of this submission will be sent to you. Another copy will be saved on a spreadsheet made available during the planning of the picnic. There is no limit to the number of nominations someone may receive; only the best submissions will be awarded.
The nominee does not receive a copy of this at the time of submission but may receive a copy of the submission at a later date regardless of whether the award was awarded or not.
